Chella Chiropractic, LLC ("Chella Chiropractic," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, share, and protect information when you visit chellachiropractic.com, book an appointment online, contact us, or receive text messages (SMS) from us.

As a health care provider, we are a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Health information we create or receive in the course of your care is protected health information (PHI) and is governed by our Notice of Privacy Practices, which you can request at our office or by contacting us. 1. Information We Collect

Contact information: name, email address, mailing address, and phone number, including mobile phone number.

Health information: medical history, current conditions, symptoms, insurance details, and other health information you provide through intake forms, online booking, or communications with us.

Appointment and billing information: appointment dates and times, insurance information, and payment details. Online payments are processed by our third-party scheduling and payment providers; we do not store full card numbers on our website.

Communication records: messages you send us by email, phone, contact form, or text message, and records of your text messaging consent and opt-out requests.

Website usage information: browser type, device type, IP address, pages visited, and similar data collected automatically through cookies and analytics tools.

2. How We Use Your Information

We use your information to provide, coordinate, and document chiropractic care; schedule appointments and send confirmations and reminders; communicate with you about your care, forms, and account, including by text message if you opt in (see Section 3); bill for services, submit insurance claims, and process payments; respond to your questions and requests; operate, secure, and improve our website and services; and comply with legal and regulatory obligations.

3. SMS / Text Messaging

Chella Chiropractic offers text messaging to patients and prospective patients who opt in. This section applies to all text messages we send.

Consent: You opt in by providing your mobile number and checking the text-messaging consent box on our intake form, online booking page, or website contact form, or by giving consent at our office. Consent to receive text messages is not a condition of receiving care or purchasing any service.

Types of messages: We may send appointment confirmations and reminders, scheduling and rescheduling notices, requests to complete forms, billing and account notices, responses to your inquiries, and office updates (such as holiday hours or closures). We will send promotional or marketing messages only if you have separately agreed to receive them.

Message frequency: Message frequency varies based on your appointments and account activity.

Cost: Message and data rates may apply, depending on your mobile carrier and plan.

Opting out: Reply STOP to any message to stop receiving texts. You will receive one final message confirming your opt-out, and no further texts will be sent unless you opt in again. You may also opt out by calling 860-812-0333. To opt back in, reply START.

Carriers: Mobile carriers are not liable for delayed or undelivered messages.

Health information in texts: Standard text messages are not encrypted. We limit the health information included in texts to what is needed (for example, appointment date and time). By opting in, you acknowledge this. You may ask us to contact you by another method at any time.

No sharing of mobile information: We do not sell, rent, or share your mobile phone number or SMS opt-in consent with third parties or affiliates for their marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except service providers (such as our messaging platform) that help us deliver messages and are bound to protect your information.

4. How We Share Your Information

We do not sell or rent your personal or health information. We may share information only with service providers that help us operate, such as our scheduling, electronic health record, payment, website hosting, and text messaging providers (providers that handle PHI sign business associate agreements as HIPAA requires); with insurance companies and payers to process claims, as permitted by HIPAA; as required by law, court order, or public health or safety requirements; and to protect the rights, property, or safety of our patients, staff, or others.

5. Cookies and Website Analytics

Our website, hosted on Wix, uses cookies and similar technologies to keep the site working and to understand how visitors use it. You can block or delete cookies in your browser settings, though some features may not work properly. We do not use website tracking tools to collect or disclose health information for advertising.

6. Security of Your Information

We use reasonable administrative, physical, and technical safeguards to protect your information. Online forms and booking pages that collect health information are encrypted in transit. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

7. Your HIPAA Rights

Under HIPAA, you have the right to request access to and a copy of your health records; request corrections to your records; request restrictions on certain uses and disclosures of your information; request confidential communications, including asking us to contact you by a method other than text message; receive an accounting of certain disclosures; and receive a copy of our Notice of Privacy Practices.

8. Children's Privacy

We treat patients of all ages. Information about minors is provided by and managed with a parent or legal guardian. Our website is not directed to children under 13, and we do not knowingly collect information online directly from children under 13. Text messages about a minor's care are sent to the parent or guardian's mobile number.

9. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. Changes will be posted on this page with a new effective date. Material changes to our text messaging practices will not apply to you without your consent where required by law.
